Recognizing the critical importance of health and mental health care to individuals, families, and communities, the Foundation invests in programs or projects that achieve results aligned with the objectives outlined below. Increase the capacity of New Yorkers to be active and educated health care consumers who are able to make informed choices about their own health and effective use of the health care system.

Results Sought:

For nurse recruitment and retention and to design and launch a clinical rotation program that will increase the number of nurses interested in the NYC Nurse-Family Partnership

The Institute for Family Health, Inc.
To help enhance the usability and usefulness of MyChart-MyHealth, the Institute’s electronic health records system, and develop a related patient training program that will enable patients to access and understand their health information

Literacy Assistance Center, Inc.
To help support the Health Literacy Resource Center
